Auto Accidents:

What was the date, time, and place of the auto accident.

Who was driving each vehicle, and were there any passengers or witnesses.

Was there a police report.

Was there any insurance, and if so, was it “limited tort”

Were there any personal injuries, if so, list them along with the names and addresses of your health care providers.

Was there any wage loss, loss of earnings or lost earning capacity.

Were there problems with the road (ice, snow, or pooled water, pot hole, or no guide rail, etc.), or with the car (bad breaks, defective seat belt, roll over, etc.).
